As a Clinical Pharmacist, you’ll play a key role in advancing medication adherence and improving quality outcomes for patients we serve through Medicare and value-based care programs.<br><br>This role is ideal for a pharmacist who is energized by population health, passionate about patient engagement, and motivated by measurable impact. You’ll partner closely with providers, Healthguides, and care teams to close adherence gaps, improve HEDIS performance, and help patients achieve better health outcomes.<br><br><strong>What You’ll Be Doing<br><br></strong><strong>HEDIS & Quality Improvement<br><br></strong><ul><li>Leading initiatives to improve HEDIS medication adherence and pharmacy quality measures, including adherence to antihypertensive, cholesterol-lowering, and diabetes medications. </li><li>Analyzing adherence and pharmacy data to identify gaps in care and designing targeted, scalable interventions. </li><li>Partnering with providers, Healthguides, care managers, and internal teams to drive quality performance in a value-based care environment. <br><br></li></ul><strong>Patient Engagement & Clinical Interventions<br><br></strong><ul><li>Conducting Medication Therapy Management (MTM) sessions to uncover barriers to adherence and optimize medication regimens. </li><li>Educating patients with clarity and empathy around medication use, side effects, and dosing schedules. </li><li>Applying motivational interviewing techniques to support behavior change and sustained adherence. </li><li>Addressing real-world challenges such as medication cost, side effects, and regimen complexity. <br><br></li></ul><strong>Data Analysis & Reporting<br><br></strong><ul><li>Monitoring and tracking HEDIS performance metrics and evaluating the effectiveness of adherence programs. </li><li>Using EHR and pharmacy data to proactively identify patients at risk for non-adherence and developing actionable care plans. </li><li>Preparing and sharing insights with leadership, translating data into recommendations that drive continuous improvement. <br><br></li></ul><strong>Collaboration, Training & Subject Matter Expertise<br><br></strong><ul><li>Serving as a medication adherence subject matter expert (SME) across Guidehealth. </li><li>Supporting and educating pharmacists, Healthguides, care managers, and clinical partners on adherence best practices. </li><li>Participating in cross-functional initiatives focused on quality improvement, population health, and scalable value-based care solutions. <br><br></li></ul>Qualifications<br><br><strong>What You’ll Need To Have<br><br></strong><ul><li>Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy. </li><li>Active, unrestricted pharmacist license in Georgia (GA). </li><li>2+ years of experience in HEDIS medication adherence, quality improvement, or population health. </li><li>Strong working knowledge of HEDIS measures, particularly medication adherence metrics. </li><li>Comfort working with EHRs, pharmacy data, and performance reporting tools. </li><li>Excellent communication, analytical, and organizational skills. </li><li>Ability to work independently while thriving in a collaborative, remote-first environment. <br><br></li></ul><strong>Would Love for You to Have<br><br></strong><ul><li>Experience in managed care, pharmacy benefits management (PBM), or healthcare quality improvement. </li><li>MTM certification, Certified Geriatric Pharmacist (CGP), or similar credentials. </li><li>Familiarity with value-based care models and how quality measures drive outcomes. </li><li>Experience using motivational interviewing or other patient engagement methodologies. <br><br></li></ul>Additional Information<br><br>The salary range for this role is $110,000.00 to $130,000.00 per year<br><br><strong>ALIVE with Purpose: How We Thrive at Guidehealth <br><br></strong>At Guidehealth, our values come to life in everything we do.<br><br><ul><li>We are Driven by Accountability — grounded in transparency, reliability, and integrity as we navigate challenges and opportunities alike. </li><li>Always Growing, Always Learning — staying curious and continuously improving inspires us to shape a better future for healthcare. </li><li>With Collaborative Innovation, we solve problems creatively, making every experience better for our employees and the patients we serve. </li><li>At Guidehealth, Every Voice Matters — we believe our collective strength is rooted in the unique perspectives of each team member. </li><li>And through Empathy in Action, we build stronger connections with those who count on us. </li><li>This is what it means to be ALIVE with purpose. Temporary employees and contractors are not eligible for benefits.<br><br><strong>COMPENSATION:<br><br></strong>The listed compensation range listed is paid bi-weekly per our standard payroll practices. We provide new employees with the necessary equipment to function in their role at no charge to the employee. Employees provide their own internet connection, capable of conducting video calls on camera and connecting to various internal and external systems.
